Bethesda developers faced a massive technical hurdle while building the Broken Steel expansion for Fallout 3 because their outdated game engine had absolutely no idea how to render a moving vehicle; rather than writing complex new code from scratch, they creatively attached a massive, highly detailed train model to the head of a hidden, underground non-playable character, and then just made that digital mutant run incredibly fast along a predetermined track beneath the map; the player is technically just riding inside a giant piece of headgear while a furiously sprinting subterranean humanoid carries them toward their destination, perfectly highlighting the absolute unhinged madness of triple-A game development, where the smartest solution to a multimillion-dollar software problem is to just invent a guy with a subway car for a hat
