Soyuz 11 was the only mission where people died in space
Despite all the dramatic cinematic explosions, the edge of the atmosphere is relatively safe—unless you count 1971. The three-man crew of Soyuz 11 are the only humans to perish above the Kármán line. A faulty breathing valve depressurized their capsule during descent. They literally drowned in the freezing vacuum of space, which is a horrifically grim reminder that the universe actively wants to kill us.
