The screwdriver that held a plutonium core apart

Safety protocols in the 1940s were apparently just mild suggestions. Physicists literally kept a 14-pound subcritical sphere of plutonium from going prompt-critical by holding a beryllium reflector apart with a standard flathead screwdriver. Naturally, the screwdriver slipped, triggering a blue flash of doom and blasting them with a fatal dose of radiation.
