Organ transplants were a spectacular failure for decades because the human immune system is aggressively xenophobic to foreign tissues. The breakthrough finally happened in 1954 purely because the patient and the donor were identical twins. The doctors essentially tricked the recipient’s immune system into thinking the new kidney was its own, successfully bypassing the biological security guards entirely.
